    The clinical pharmacy team work as an integral part of the team that manages the care of the patient in the home. They liaise closely with medical and nursing staff within the referral hospitals and where relevant the medical department of the pharmaceutical companies. In addition, they provide valuable input into weekly clinical round meetings, monthly clinical governance and on-going service audits together with development of Pharmacy related SOPs and Forms in conjunction with our Quality Department.
