Benefits:

  • TCP Homecare have built a new patient referral portal to facilitate the referral of our training and education services​​​​​​
    • This has been done to reduce the complexity of our referral process, and save the HCP time.
  • The HCP will be able to refer to multiple services from the same account.
  • The HCP will be able to see all patients from their hospital and unit
    • This is to create greater patient visibility at the unit level to ensure transparency of care
  • The HCP will be able to switch existing patients to new services at the click of a button.
    • The switch feature has been established to reduce the referral time for the HCP.
  • TCP is pushing changes to improve our patient engagement.
    • We are rolling out the availability of virtual app calls for these services to improve patient engagement. 
    • We are rolling out the availability of patient self-referrals for Waste-only visits

You can see any patient that has been referred for your Hospital and Unit for any service that is on the portal.

What can I do if my forgot password link is not working?
